2022-2024 Comunity Health Assessment/ Community Health Improvement Plan

Community Health Assessment

Every three years, CCHD and the University of Vermont Health Network - Champlain Valley Physicians Hospital (CVPH) develop a Community Health Assessment (CHA) to identify and prioritize the community health needs of Clinton County residents. We do this to better understand the factors and root causes that contribute to higher health risks and poorer health outcomes.

Clinton County health partners selected the two following priority areas and disparities as most imperative for Clinton County to address as a community over the next three years.

  • Prevent Chronic Disease
    • Healthy Eating and Food Security
    • Physical Activity
    • Chronic Disease Prevention Care and Management
    • Tobacco Prevention
  • Promote Well-Being and Prevent Mental and Substance Use Disorders
    • Promote Well-Being
    • Prevent Mental and Substance Use Disorders
  • Disparities
    • Income
    • Geographic Location
    • Age

The CHA is used to inform strategies and plans to enable all community members to attain their optimal health; that planning is summarized in the Community Health Improvement Plan (CHIP).

CHA Resident Survey

Clinton County residents are invited to participate in the CHA process through a survey. Residents are asked to provide feedback on health, social and environmental issues in the community and to report on the challenges they or a family member has experienced. Demographic information is collected to ensure a representative sample of residents is reached.
