Family Health Home Visiting Program (FHHVP)

The Clinton County Health Department is committed to helping families. The Family Health Home Visiting Program (FHHVP) offers services to expectant parents and new families. The program is flexible and can be tailored to meet the needs of the family.

The FHHVP nurse will provide information about community services, parenting, normal pregnancy, dental care, nutrition and healthy lifestyle choices.  After your baby arrives, the nurse will answer questions and concerns about such thing as feeding, safe sleep, immunizations, growth and development and when to call your doctor.

Prenatal

One of the most important things you can do for your baby when you are pregnant is to get early and regular prenatal care. The FHHVP nurse partners with your doctor to offer help to expectant mothers about being pregnant, becoming a parent, and your medical care. Please contact your doctor as soon as you think you may be pregnant.

Postpartum

Becoming a parent is one of the most exciting times of life, and also one of the most stressful. The FHHVP nurse listens to your concerns as you adjust to parenthood and answers your questions and concerns.

 

Who is eligible

  • This is a free service to all Clinton County residents.

How to get started in the program

  • Refer yourself by calling the Clinton County Health Department (518) 565-4848
  • Your doctor or provider may send a referral if needed
  • Other community agencies may send a referral if needed
