WHO DOES WHAT?

Partner Nurse Program (PNP)

Clinton County Health Department’s (CCHD) Partner Nurse Program (PNP) connects local health care professionals in clinical PartnerNursepractice to those providing community preventive services. In doing so, the Partner Nurse Program strengthens communication and collaboration across the healthcare system for better health outcomes of all residents of Clinton County. 

Quarterly, a CCHD Nurse makes face to face detailing visits, providing local medical practices tools and resources that support high quality patient care. The PNP Nurse is there to serve as a point of contact, familiar face and link to additional resources.

Services and items commonly provided through the Partner Nurse Program include:

  • Updates about revisions made to clinical practice guidelines;
  • Evidenced based literature related to emerging public health issues and health issues of local or seasonal significance;
  • Population health data trends and analyses;
  • Progress made in the County’s Community Health Improvement Plan (CHIP);
  • Health messaging and education materials for waiting and exam rooms;
  • Information on CCHD services and activities as well as local opportunities for health improvement;
  • Details related to professional development, continuing education and emergency preparedness activities within our region.
